HOSPITAL seems certain.
Since the clue isn't, say, Infarction, I think UNDERTAKER and WAKE are unlikely.
CODE is possible, that's what hospitals say when the patient needs to be revived
NEEDLEs are used in hospitals, also to inject drugs into the heart to save a patient from overdose
The clue wasn't Heart, so not LUCK or STRING
An x RAY is used to check for heart problems or damage

Post spoiler: as stated, I disagree with UNDERTAKER. Cardiac just means "concerning the heart," I believe, so doesn't imply death.

Cardiac problems can put you in HOSPITAL, and acute heart conditions (Cardiac infarctions and/or arrests) have specific CODEs in American hospital dramas when announced over the tannoy (I don't know about real life, happily)? The staff may use NEEDLEs into the heart to try to make it behave.

And if it does all lead to unrecoverable Cardiac arrest you get an UNDERTAKER involved and are possibly guest of honour at a WAKE? But the link between UNDERTAKER and WAKE feels very strong to me - too strong for only one of the words to be in the set, and I think they could have been clued differently (Morgue for HOSPITAL + U + W for example).

HOSPITAL > CODE + NEEDLE > UNDERTAKER + WAKE
