Sorry, timing has not worked out well for me this evening - I'm going to post my thoughts, but am going to have another stab at this tomorrow morning, and will wait until then before even thinking of pointing.
Before I start: with NEEDLE and RAY both gone I would have seriously re-considered my earlier dislike of UNDERTAKER / WAKE for Cardiac, even without having seen this clue.
We're outside the spoiler zone, but these thoughts were drafted before reading the other posts.

So, the ferryman of the Styx (or possibly the Acheron? Greek mythology is not always self-consistent), after whom Pluto's SATELLITE is named. Anyway, ferries dead souls about (and takes the coins that were put on their corpses eyes as a fee IIRC, but I can't see any links to that here).
UNDERTAKER - strictly, deals with souls not bodies, but yeah
WAKE - the wake of his boat
WINDs his oar on a one-way TRIP (for his passengers)?
A cloaked FIGURE? At least in a lot of art (I was reading the Wikipedia article on Dante's Inferno last week).
That's six, so one too many. I can also vaguely see links via Greek mythology to ATLANTIS, but that's a stretch.
SATELLITE + UNDERTAKER > WAKE + TRIP > FIGURE > WIND for me
Is El Grillo may be steering us away from CODE, with WAKE + UNDERTAKER here?

Despite my comment, I think Cardiac = HOSPITAL, UNDERTAKER, CODE is plausible - but I'd wait to see if WAKE is blue before trying CODE personally. Wouldn't rule CODE out, but would make me pause.
Personally, I don't like Bermuda for this. To me, it's a holiday destination and tax haven, but the "triangle" thing may be more a common view.
And his boat probably has a shallow DRAFT, just to throw one more thing into the mix.
