I actually didn't make any further adjustments to the starting location script other than increasing the starting distance to 30 minimum. My previous change forcing player starts to be fresh water didn't have any effect on the city state starting locations anyway. I rolled a couple of small Pangea maps with 2 players and the max number of city states (14). Here are two examples (player starts identified with red circle):
The city state distribution looks pretty good. Also I fixed the issue with the Aztec DLC so that is an option now. My civ pick:

There are 14. It looks like I forgot to set the difficulty to Diety, it’s showing Prince on the loading page.
These first-to-meet envoys are very un-balancing to the early game. I think I see why you were asking how many CS there were, I've just met my 10th now. I think it would be advisable in the future to have someone roll the map and assign the starting locations to guarantee that they are distributed somewhat evenly.
