A save game with a "violation" would also help.
From the save game I can find for instance (after the fact) what the raw item value was in the particular lair.

In v1.31 AI won't create items.
I think Insecticide does, but I guess we'd need to test it to be sure, monitoring what the other wizards are casting.
Next problem: the maximum item value that will be generated is 2500.
That means that (in v1.40n) 54 of the 250 items cannot be found in a lair.
And even if AI would cast create artifact, there is no prefab item it can select, since no item has a cost of 30000 / 2 = 15000 or more.

Note that it says that I have an "unseen" item of cost 8200 "InGame".
That means some opposing wizard somehow acquired that artifact.
It could be a fluke of course, but I see more items like this that are "InGame" making me wonder.
As far as I know, the only way an opponent can get an item like this, is if the merchant offers it.
And yes, the opponents have a pretty big probability of being offered items like this.
Actually, that might be an explanation.
What if all extreme items have been offered to AI wizards before you get a chance at them?
This lair with a zero book wizard looks interesting. I'm going to run it all night, just to see, but so far the only special items I am seeing are Flaming.

I've found the problem after eliminating all other possibilities.
The ITEMDATA.xls I had uploaded is actually from Insecticide.
I was not aware that kyrub had changed it.
It turns out that the Double Axe of Pain does not have spell book requirements in v1.31.
I have updated the link. For reference, that is ITEMDATA.xls.
It now contains 2 tab sheets. The first is the list of v1.31 and the second is from Insecticide 1.40n.
Looking through the v1.31 list, I can see there are 16 items that cannot be found in a lair because they require more than 13 books.
Monster Masher is an edge case that requires 13 books, and there are 4 more items that require 12 books.
As far as I can tell, all items should be available through the merchant, except 2: the items with merging.
With your 0-book strategy in v1.31 you can find only 91 items, and the only special ability they can have is Flaming or a spell.
Let me know if you find an item with a special other than Flaming or a spell.

Seeing that the least frequent item was found 24 times, and that these 9 were not found at all.
It looks as if this result should be statistically impossible.
