Yes, that will be the difficulty when the game gets started. Difficulty isn't set as part of the map making process, but is set when the game is created.

We did not ban vassal states (and consequently colonial maintenance), did we?

I would have thought that "no vassal states" was just assumed to be checked on unless noted otherwise, along with things like "unrestricted leaders" and "no tech trading" and all the victory conditions and "no random events."
