I'd also prefer a map on the larger side, for the reasons naufragar outlined.
Pretty much the above, except I prefer 2.0.8.4 as I like the idea of putting in the work for the maps. Also opens up the spy game a bit more.
I am fine with 150-170 per civ if 200 is too large for some. But not a dealbreaker, whatever is eventually chosen.

With a smaller roster of players, OBs will likely be harder to score. So refreshing an NAP every 10 turns loses the 100% continued peace bonus if you happen to have open borders with that civ. Otherwise set at 10 turns OR successful city conquest if before then, to allow for wars of opportunity. Sometimes you just need that one city for now.

I guess I prefer a map on the bigger side. Reduces variance.
Personally I'd prefer not to give too specific instructions as I'd rather find out what the map is like playing the game. Do try to achieve some sort of balance, nobody should be lost on T0, but it doesn't have to be mirrored starting areas as far as I'm concerned. More so with a larger map. Just make sure you don't use global highlands.
In short: I'm not too bothered and will go with the consensus.

Mardoc, my ideal is that someone would do the following:
1) Roll a few medium / low sea level Big and Small cylindrical maps.
2) Choose the most reasonable of them.
3) If even in the most reasonable one someone is totally screwed by a start, connect him to a nearby large island or something like that.
4) Make sure that everyone has key strategic resources somewhere close.
5) THE END.

I don't have a preference about which map script you use, just make a map that you think will be fun to play and where nobody loses T0 due to map reasons.
Being a PB11 alumnus I'll always err on the side of preferring more balance rather than less from the map I'll spend six months playing on, so I'm willing to wait if the map maker has the time and inclination to improve the map.
I'd prefer RtR 2.0.8.4 over .3 for the same reason DZ specified.
For size I'll defer to the map maker. Keep it resaonable/playable for the end game.

I can do this, with the proviso that I would normalise the starts so they are all similar in output (but not in resource type). eg they would all be double food starts with a river, or all single food starts with a production tile for similar outputs. None of this triple fish for one and single plains cow and a billion forests for another. Just remember that this was what was played on PB8 when Serdoa got landed in the middle of a fuck off huge jungle and couldn't interact with anyone for 120 turns (actually, I deleted a bit of jungle to make it more playable, the original map was even worse).

PB8 was also one of the most entertaining games played here (I actually moved from lurking to posting at that time). So, fuck yeah!
