1) Are both your son and the Duchess 16 years of age or over? Once they're 16, there should be an alert at the top saying "betrothed can marry."
2) If you have Low Crown Authority or higher, you can change formation leaders. However, this is not possible when you're an independent duke and there is no King of Ireland to raise crown level on his own.
Also, be aware that depending on how the Duchess's lands get inherited (who dies first, and only if you aren't a king), you may end up as a vassal of England in the future.

They're both 19. I got the alert with a previous marriage, but not this one. The lad is not directly in line for inheritance, he has two older brothers (one already an adult at the game start, and the other I groomed as my treasurer) so I'm not worried about vassalage.

Then propose a regular marriage between the betrothed couple. Unless she got married somehow.

Tried that, she wasn't available on the screen. And she no married neither. I even tried going over her head to the King of England. It baffles me.

Yes, I annexed the Mayoralty of Limerick to give him. A son that good you have to give special presents to.

That could be the problem. He's your vassal, but not actually in your court. He can choose his own marriage.
First rule: wed your heirs before you land them.
