FOCUSING JUST ON NEIGHBORS, here's how I'd place 'em (islands and/or bulges for extra connections are green lines):
   
Alhazzard is the only one who screams "rougher than warranted" to me, and he does have best access to the Tasty Trio. Give him one of the more militaristic combo sets? Molach also would need some decent fighting leaders/civs, but I was impressed by his land-holding in PB18.

Edit: I could also see shifting Ipecac to his south coast.
If only you and me and dead people know hex, then only deaf people know hex.

Player in middle of 5 player continent should be the best player out of those 5.

IMHO that's the same as making an entirely new map. And yes, it really is a lot of work.

This setup could work, but it requires a lot more map editing:

1.) Swap REM into the middle (player 0's slot) and Dantski to the NW (player 3's slot). Dantski can't be in player 5's spot (where you put ipecac) unless that whole southern/central desert chokepoint is entirely reworked.
2.) Buff Dantski's spot
3.) MASSIVELY buff Alhazard's spot
4.) Gav's spot needs to be buffed for real
5.) I think an island to OH's south and 2mn's east would be interesting

It would also be great if TW's whole area could be shifted NE about 5 tiles.

This is on top of the other additions I mentioned earlier. I think your previous suggestion was much better and requires way less editing of the map.
